Muse can't support server-side code which you will need to gather and process form data. 

Use a 3rd party service like Wufoo.com and embed their registration form into your Muse page.

yes it is possible

the service I use is https://www.publiz.com/ but there are a few others... the big issue (as Nancy said) you need some kind of cms and the rules change from one country to the next i.e, UK and Australia have different laws to the US
