Photo Submission widget for Adobe Muse?

Is it possible for people to upload a photo to my website within the simple contact form?

We have salons and are always looking for models, would be nice if they could upload a selfie/reference photo to be included with their contact info, is this possible?

This feature is not currently available in the form widget in Adobe Muse. Have a look at these widgets to see if any of them meet your requirements:
