Go to Edit>Preferences>Tools and check Vary Round Brush Hardness based on HUD vertical movement.

If the brush is not one of the default round brushes then the Alt drag up/down shortcut adjusts the opacity regardless of the above setting.

In Edit > Keyboard Shortcuts you can remap size and softness.

Brush opacity is controlled by typing numbers directly. If you want 20% opacity, you just press 2. If you want 25%, you press 2 and 5. And so on. Full opacity 100% is 0 (zero). This works automatically with a brush tool selected and active.

@陳風 Wind Chen I assume you mean the ALT right click to adjust the hardness, Photoshop is currently on version 25.4, have you tried resetting the tools

It's so easy to forget the basics, and try to over complicate things with Photoshop.  Plus Photoshop's Preferences have evolved, so it's always a good idea to have a good trawl through Prefences after major updates.
