Select the Brush tool, and click on the Symmetry icon in the Option Bar (the butterfly icon)

Horizontal is one of the options.

Sorry.  I have just noticed that you are asking about Photoshop on the iPad, and there is no option in the iPad version.  I have an idea, so let me test it and get back to you.

I am just not very good at using the iPad, but if you have a Windows or Mac computer or laptop, try creating a document with symmetry and saving it as a PSD file.  Email it to your iPad and open in Photoshop.  Try using the paintbrush and see if the symmetry still works?
