Cursor takes square shape & app freezes when Add Noise panel is opned. How to fix it? Video is uploaded.

What version of Photoshop and Windows are you using?
The square cursor is as it's supposed to be so one can click on areas in the actual image and then the preview screen in Add Noise moves to the same view.
The difference is when everything is working the cursor should change to a Hand when hovered over the Add Noise preview windows so one can scroll the preview.
Other filters with a live preview window such as Filter>Blur>Gaussian Blur behave in the same manner.

If you try other filters with a live preview does Photoshop freeze?
Perhaps a reset of the Photoshop preferences will fix the issue:

That's normal behavior in some dialogues. Click on the image with the cursor (with Preview checked) to see a preview of that part of the image in the Filter dialogue.
