inactive PS screen, unable to close without forced closing using task manager

when  I open a photo, on the left side of the upper ribbon of the photo reads camera raw 14.5, and on the right side  I have an icon for settings (it does not provide any icon to let me exit this particular screen) and for download/save. There is no icon anywhere for me to close this window. I have to do an alt-ctrl-del to get to the task bar to close PS completely. 

 

It is worth noting that when I first open the photo, a small PS window opens up. This window contains file, etc on the upper ribbon, but the photo opens up immediately and I cannot access any of the commands of the first window that opens up. 

 

Any ideas on how to fix this problem? Many thanks

Could you please post screenshots with the pertinent Panels (Toolbar, Layers, Options Bar, …) visible? 

 

What have you done for all-purpose trouble-shooting so far?
Restoring Preferences after making sure all customized presets like Actions, Patterns, Brushes etc. have been saved might be a good starting point:
