Keyboard shortcuts dissapear

As part of my PS workflow, I always create a minimum of two keyboard shortcuts, flatten image and restore my favorite workspace. These are in a special keyboard set which I'm presuming is activated when I see it listed in the keyboard shortcut interface. My problem is this: after I create the shortcuts, they work once and then dissapear. This also happened a couple of versions or so back. If  I solved the issue, I'm not sure of what I did. Any ideas on how to keep the shortcuts working?

Are other shortcuts part of the Workspace?
