layer transform and object moving error and brushes usage error

wheneever i tries to , resize a image or soemthing a patch of pixels show instead of image and im unable to see the actual image i just sees a patch of pixels only , and same happens when i uses clone stamp tool or brush tool ,check photos or videos to understand the bug more deeply !

We need more info to help you. What version of Photoshop and operating system are you using? Does this happen when you open any specific file?

It is always recommended that preferences be backed up before resetting it.
