I don't see any direct problem with Photoshop at the moment. It looks like more that's a picture with a higher ISO setting. Which ISO setting do you have used in the camera.

How looks the picture in CameraRAW?

My System: Intel i7-8700K - 64GB RAM - NVidia Geforce RTX 3060 - Windows 10 Pro 22H2 -- LR-Classic 13.2 - Photoshop 25.6 - Nik Collection 6.9 - PureRAW 4 - Topaz Photo AI 2

You must view at 1:1 (100%) zoom to even evaluate noise/grain so are you doing this?

Be sure to view both the preview in ACR then the resulting image in Photoshop at that zoom ratio.

Also, if you can upload an example raw to something like Dropbox, we can view this and I can even analyze the raw data another application to examine this noise prior to rendering in ACR.

See the solutions here: https://community.adobe.com/t5/photoshop-ecosystem-discussions/pictures-suddenly-look-grainy-in-phot...

It's either a GPU Driver issue or Cache Levels set in Photoshop's preferences.

Many cameras will show some noise at ISO 4000. Does it look OK in Camera raw when viewed at 100% zoom?

Please show a screenshot of Camera raw at 100% zoom and Photoshop at 100% zoom

Until you provide the raw for examination, there's nothing you've provided that indicates this is any kind of bug in the product. At ISO 4000, depending on actual exposure (which, again, I can evaluate for you), there is no reason to believe this noise is anything but the result of the raw data.
