photoshop glitch?

so i've been having a problem with photoshop glitching? It looks like this:

and it seems to 'trigger' when i zoom in, zoom out, create a layer, hide/unhide a layer, and use the fill tool. it started yesterday night and hasn't let up, even after updating to the latest version. it only happens with photoshop. what is going on?
