Saving a PDF but it loses elements

I can save under any other format and everything is there when I reopen it, but when I save in PDF there are missing elements and my design is incomplete.  Unfortuately I need the PDF as the platform I am uploading to only accepts that type of file.  I have updated Photoshop this morning, opened and closed the program a few times, but still having the same issue.  Does this happen to anyone else?

Can you confirm you have "Layers" checked in the initial save dialog box and then "Preserve Photoshop Editing Capabilities" in the second window?

If possible, a copy of the source PSD and the resulting PDF might help us troubleshoot faster. (link to CC files or dropbox)
