Spot Healing brush not working since update

I updated to PS  recently to latest version with all updates. My spot healing brush now 

no longer works properly. I don't get the quality of match but rather almost a blur or strange pixelated spot that stands out so I cannot even use it. I have to us the clone stamp tool. Not sure how to fix this or what is cause as this just started with  the newer version. I've done all updates and it still doesn't work right.
