Is this only happening with type layers?

What are your anti aliasing settings for type?

The only one that would cause real problems if it were set to None

If it happened with drawn lines, then you'd check to see if you were using the pencil (which has zero anti aliasing) as opposed to the brush tool.

How far are you zoomed in?

At zoom levels greater than 100% you are going to see the document pixels and these will show up on diagonals close to horizontal / vertical as steps. Smoothing fills some adjacent pixels with intermediate values but the limit is still there. Photoshop is a pixel editor and even vector based shapes such as text, can only be displayed using the minimum unit of a document pixel.
