when i save a file, file gets screwed up

when i save a fiel in photoshop as a png or jpg , something gets messed up in image...an added blue box over part of it or an image missing...etc...i have to try a few times before it is ok. it is happening more often now and i am afraid wont work at some point.

Just to confirm your steps:

Open the attached nyckidsrise_big.psd

File > Save a Copy... as JPEG or PNG

Or are you using Export As or Save for Web?

I can't seem to reproduce in any of the File > Save or Export paths.
