Photoshop Mix Multiplying Image in Itself

I really don't know how to even word this issue but I will try my best to describe what is happening.

I am attempting to add an Image to my project.  When I select the Image it duplicates it 4 times as well as distorting the size and shape of each copy differently. 

The duplicates aren't created in different layers, they are all within the same layer.  To make this even more confusing, I went to a previous project that I had used the same image in and everything looks fine, except when I go to open it up and edit it, the Image duplicates itself and distorts as well. 

I have tried opening the image in different applications and there is no issue.

I am absolutely baffled.

Couple of questions:

1. Please let me know your device information on which you are using the application.

2. Are you inserting a new image layer before bringing in the additional image in your project?

3. Have you tried to uninstall and reinstall the application?
