Bug With Healing Tool

Hi,

I'm a film photographer and i came to you for some problem with Healing Brush Tool and with Spot Healing Brush Tool. With Photoshop 21.1.3

When i want to clean my picture i set up the tool :

1-Mode : Replace

2-Source : Sampled

3-30% or lower opacity

And, the tool doesnt work properly. Because the content is not good, blurry and the tool doesnt respect the films's grain (see the attachment).

I have tried with different opacity and different Type (for Spot Healing Tool) but the problem still here.

In left, the original file. In right, the same but after the Healing Spot Tool (same with Healing tool)

I dont undestand why this is not good 😕

Any Idea ?
