Creating a fade-out with gradient but still shows an edge and no smooth fade into transparency

I am trying to use a mask in a layer and a gradient to fade the image out into transparency. However no matter what I do, there is still a vertical edge visible. I managed to coincidentally get it right, but I can't reproduce it.

Any help is welcome....

Could you please post screenshots with the pertinent Panels (Toolbar, Layers, Options Bar, …) visible? 

And set the fore- and background colors to black and white.

Please look at the Gradient you chose in the Options Bar – it does seem to be neither black-to-white nor foreground color-to-background color.
