What is an »overlay action« supposed to be exactly? 

Please post a meaningful description and post screenshots taken at View > 100% with the pertinent Panels (Layers, Channels, Actions (?), Options Bar, …) visible to illustrate.

Please post screenshot as already suggested.

 

In meantime check what blending mode you have set for overlay layer or effect. Blend mode like Overlay or Soft Light should always allow another layer of overlay while Normal blending mode will override whatever is below.
