Inserting a contact form on a full page image

I have designed a Contact Us page in Photoshop, a full page image, and I want to put a Contact form on it. Sure seems to me there should be a way to do that. Please see attached Contact Us page. Any help would be greatly apprecited. Thanks

If you mean you want a "Contact Us" form that will work on the web, you can't do that in Photoshop. You can create the artwork in Photoshop as you have, but you need Dreamweaver or WordPress or another editor to write the code for the form. Here are directions from W3Schools.com:

I think there are some sites that have code that you can copy.

I have gone to Jotform and found a full page contact page. I have the
code for it, but when I drop the code in, all I get is lines across the
page. I'm trying to put the code in a new HTML page, I've also tried a
CSS page, same results. I'm obviously doing something wrong, please help!

The answer is the same: you can't do this in Photoshop. You might ask in the Dreamweaver forum if you are using DW for HTML and CSS. Photoshop is an image editor.
