The "classic" way of doing this was to use a Stroboscope flash in a darkened room on a moving subject (search for Dr. Harold Edgerton for some background on this).

 

These days, you would use a sequence of images of a moving object, as with video or continuous/burst series of stills, and then combine them into an Image Stack in Photoshop.

You might want to try the Blur gallery in photoshop as well. might not be excactly like the image but soemtimes I get great results. Just make sure you crreate  smart filer first. These filer gallery in dirctly on the screen so you have a live preview.
