Elements flips clip, looks filtered and runs into the next clip

Randomly while editing, elements will flip a clip upside down, it will also flicker and look filtered like a negative. that clip will also run into the next clip so the visual keeps playing but you can hear the next clips audio. Please help.

Where did your source video come from? What camcorder or device recorded it?
