Audio Desync issue

There are several audio desync threads, but I couldn't find one that sounded like my specific problem.

I'm digitizing some old VHS tapes for a friend using an El Gato Video Capture Card. The files created by the El Gato's software seem fine. I open them in VLC and they're perfectly synced.

When I import them into Premiere the audio (ONLY IN SOME SECTIONS OF THE FOOTAGE) is wildly out of sync. Seemingly at least 30 seconds off, maybe more. This is not the type of desync where the audio will "drift" (starting synced, and then by the end of the footage it's a few seconds off). It's suddenly and wildly out of sync at certain parts.

I tried dragging the mp4 into premiere, but I also tried manually finding it in the media browser and selecting "import" just in case that made a difference (it didn't).

Again I want to stress that the footage is fine when I play it in VLC, but to make this more confusing, the footage is ALSO desynced in After Effects.

What could be happening?
We're on a tight deadline (this is for a video to be shown at a memorial) and I am completely unsure how to even begin troubleshooting this.
