This doesn't happen too much but probably once every 2 projects. I will go to export stills or the video and it will export like this. Any idea what it is? I am running the latest version of Premiere and it's happened in other versions as well.

You're using a Media encoder to render or Premiere pro itself? let us know the image and video format and OS.
In case you're taking the output in .mp4 try changing the performance from Hardware encoding to software only under encoding settings.
