Exporting in 4k - Black dots on still opening image

When I export in 4k (2160p), the opening image a camera still shot with a title has 3 black square dots in the bottom left hand corner. The rest of the video is fine. 

Any idea why this is?

Do you see these squares in the raw footage on your disk or in your Premiere timeline? Are you saying it only appears on export? If you export that small section to a different format, do you still see the same issue? The first thing is to track down at what point these dots are entering your pipeline.
