Does anyone know of a way to get this greeting card opening effect?

I use the basic 3d effect for page turns.

I create a transparent psd double the width of the page in Photoshop. I place the page in that psd justified right and import that into Premiere Pro.

Then it will swivel as needed.

Keyframe the position if you want it to slide as it does in the video.

Changing the anchor point might work, but I haven't tried that.
