iPhone video in landscape not to scale in editing and shifts to off screen whenever I add graphics

Newbie to Premiere Pro.  I shot video on my iPhone in landscape.  When I add my media onto the timeline, it is not to scale.  I select Clip-Video Options- Scale to Frame Size.  It's still offset or doesn't use all of my video.  

Also,when I add text graphics or video graphics, it sets all the videos on my timeline slightly off of the screen.  I'm always recentering my video clips.  

When I finished and exported my project, it's a tiny output on my laptop and my iPhone.  

I've never had this happen before.  Is there a default or setting I'm missing?
