Premiere not bringing in audio track with video into new sequence

Premiere is not bringing the audio into my new sequence, even though it plays in files.... have tried restarting,  creating a new project....no change.  

Working on an old (2011) iMac maxed out with 16gb memory, running Premiere 14.8.0 (I think this is the highest version that is still compatible with this machine.).  Have run Etrecheck, report attached.  

Very weird problem. Only seems to be on this file.

The most common user error is not source patching the track.
Solved by first opening clip in Source Monitor. Then drag/insert into timeline.
