Voice-over record (microphone) option is gone in Premiere Pro 2015 :(

I have been using Premiere Pro constantly for 3 weeks, but all of a sudden my microphone button is not showing. I have tried many approaches to get it back but to no prevail. Any assistance will be greatly appreciated. 

I have checked my audio hardware settings, my customize settings and nothing helped.

Did you try customizing the header (right click)?

Where did you get this copy of 2015?

Hi, thank you for your response, I tried that this is what I see. 

I am unable to do anything here. I used to have the microphone appear on my header all the time until today morning. 

I bought this copy online.
