Adobe Premiere Pro slow performance when app in focus

So I have this weird issue in Adobe Premiere Pro where the performance is extremely sluggish.  Whether it be switching between tools, previewing video, adding graphics, etc...

 

The weird thing is if I Alt-Tab out of Premiere so that its not the application in focus it runs fine.  For example if I start previewing a video its laggy/stuttering.  While its playing Alt-Tab so its not in focus and the preview plays fine.  

 

It shouldn't be the hardware as the PC its pretty beefy spec-wise

AMD Ryzen 5950X, 32GB Ram, Samsung 980 Pro PCIE4 M.2, Nvidia RTX 3090.  Both the CPU and GPU are water cooled so its not a temp or bottleneck issue.

 

This issue is so confusing.  Also a bit frustrating as it I have to alt-tab, then alt-tab back every time i want to add or change something.

 

Tried cleaning media cache, setting up proxies, pre-render and no dice.

 

Any suggestions/help appreciated.
