Audio missing when export to m2v blue ray with Premiere Pro CS6

I cannot get combined audio with video when exporting.  Instead, I get separate video and audio files.

When I play the video in Premiere Pro, I hear audio.  I am using VLC media player to play the export file.

How do I set the export to combine both?

2 files is correct, since an authoring program uses separate files to build a disc

I know that at DVD resolution there is an option to create a single file... since I don't do Blu Ray I don't know if there is a similar option
