Images placed in the timeline will JUMP in size and enlarge but the SCALE will not change. This usually happens if an image needs to be scaled up or down or moved in anyway. Also, a usual suspect is a crop or a drop shadow placed on the image as well.

INCREDIBLY frustrating!

SO.... this has been an issue with Premiere for YEARS and I can never find a reason OR a solution. I always have to either work around or change what I am editing. Has anyone else dealt wth this?

Never. So a few questions.

What size are the images you are working with in pixels WxH?

What is your default preferences setting for timeline scaling set to ... none, set to framesize, scale to framesize?

Are there any other things you're doing such as Track Matte or something that can affect this? Nesting, whatever?
