Video not importing Audio Only (ive changed nothing)

Ok so the video plays just fine in the source panel, but if I try to drag across only the audio appears.

 

I have not changed anything with the video format I use the same camera the same sd cards the same way I have always done it.

 

the weird thing is that I imported the 1st video clip ok then when the next clip was to be added that's when the problem occurs.

 

*update* if I close the program and relaunch it seems to reset

Most common user error is not source patching the track.
Solved by first opening clip in Source Monitor, set in and out point. Then drag/insert into timeline.
