Why are 4K files not showing up correctly?

I'm importing clips into a sequence that is 3840 x 2160, and the clips match that setting.  Yet some clips do not show up in the Program window correctly.  When I try to either use Set to Frame Size or Scale to Frame size, nothing changes.  And when the file is output, even to 1080p, the clip is proportionately incorrectly sized at that format as well.

How were these recorded? Do they look normal in a video player? Is it possible that they were recorded this way?
