Hidden Files - How does it happen?

We have authors complaining that folders and files are disappearing.  If we go to the folder or file and click on the "Recent Changes" tab, we see messages that say "hidden by userx 7/15/2013 9:41 am."  They are commonly in a folder that the user listed was not working in and a file they did not touch.  Does anybody know what chases RH to make a file hidden?  We'd like to stop inadvertently hiding files and folders?  And what might be the best way to "unhide" them without introducing more errors into the folers/files?

I presume you are using RoboSourceControl as I have never come across the "Recent Changes" tab. As I don't use that application you can take this advice with a pinch of salt, but as no one else is offering advice I thought I'd chip in.

I have never heard of this issue before with any source control application. I may be worth checking the server permissions on the locations. Also has this suddenly started happening? If so, what has changed?
