I finally got all my projects updated to 2022.1 with all the output working and looking as expected.
Today, I installed the Update 2. Now the ToCs for my PDFs have page numbers for the headings on the cover page and the ToC itself, where they didn't before.
Update 1:
Update 2:
Is there a tried and true way to omit specific headings (or pages) from the ToC? The Cover page uses cover-specific classes on the H1 and H2. How can I set the ToC to exclude them (and the Contents page itself)?
Just to add on - I had the same issues when i started using rh 2022. I only encountered these issues when using the PDF templates: Prime and Hi-Tech. They kept printing ghost TOC entries also randomly repeating some chapters. I switched to basic and have not hit the issue since.

I'm not up on TOC creation in the New UI versions, but don't you have to have everything in the TOC if you want it included in the output? Then you just have to hide all the parts you don't want to appear in TOC.

Are you saying to remove the "front matter" section from the ToC if I don't want it in the ToC? How does it get included in the PDF if it's not present in the ToC?
I tried enabling "Hide in output" for the "content properties" but a) that option's available only on the Cover Page topic, not on the TOC Page, and b) that didn't resolve the issue anyway. 😛

My understanding is that it has to exist in the TOC to get into the output. But topics can be set to hide in the TOC as you've found. That should deal with the Cover topic at least. The issue with the TOC page is a bit stranger - it seems like you must show that one, because if you hid it, how would you be able to navigate to the other topics? Maybe Peter will have some ideas tomorrow. Maybe going the Word route and customizing there is the way to go?

2022 introduced templates and the cover page does not go into the TOC.
In a new project I created this structure in the book TOC, which I named Print.
The cover page comes from the template chosen, in this case I selected the PDF > Prime template. I then generated without any customisation and the cover page comes in as part of the template.
Check your set up for the TOC is similar. Then add a new template PDF > Prime and see if that fixes things.
It may not be customised as you want but that we can deal with once past this hurdle.

Thank you, Peter! That was helpful.
I've moved the cover page to the publish/template section and removed it from the "front matter." The PDF generates great -- the cover looks good, the ToC comes right after...
But the ToC still includes the cover page as page 1 (along with the blank subtitle), and the Contents showing on page 2 as shown in my "Update 2" screenshot. <head exploding emoji>

This is the area where I suspect something is wrong.

Thanks for this! I created another PDF template based on the Basic and the problems went away. (The previous templates were "converted" during the RH 2020->2022 update). I'll start there and customize.
