Is there a way to add "was this article helpful" option and get results on Rh 2019?

I'm interested in having certain articles have a "Was this article helpful?" or "Did this answer your question" disclaimer at the bottom. I'd also like there to be a writing option to populate after the user selects either Yes/No. I'd then like to gather the results. 

Does anyone have any advice on implementing this?
