Are there more settings somewhere to control pdf output other than Printed Documentation > Properties. Whatever I try, the bookmarks in pdf do not look like toc structure in help project. some head levels are not picked up.  When I Add All in Chapter Layout the result does not replicate the toc structure exactly; some h2's and h3's are left out.

Have you tried PDF Settings in the first page of the wizard?

Think I figured it out, partly the way I have my toc links setup. Some pointing to standalone pages and some within main page of chapter. However, RH still puts in duplicate toc entries for H1's in bookmarks and also duplicate titles for H1's.

I suspect part of what you seeing is where the title of the book in your TOC is the same as the first topic. See Printed Documentation on my site.
