Publishing cancelled - failed to create file

I'm using RoboHelp 9 (HTML); trying to publish webhelp to a client's server, but keep getting this error message: "Publishing has been cancelled. Failed to create file:..." followed by the name of the first htm file in my Topic list. The warning in the Output View panel of RoboHelp then reads: "Warning: Failed to publish to "NHS". reason: Canceled by user." What does this message mean?

Also, (possibly related), I read in Rick Stone's GooberGuide on RH 9 that the Start Page name must be index.htm, and that this is NOT the same thing as the default topic. Sorry if this is a dumb question, but is index.htm a file in my project that I should be able to navigate to using the Browse button (if it is, it ain't there) or am I simply to type in C:\RH Projects\Project Folder Name\!SSL!\WebHelp\index.htm? I appreciate your help.

The message is covered in Snippets on my site.

I doubt Rick said that the start page MUST be index.htm. It would be a recommendation. You enter it in the wizard as you have set out and it gets created as you generate.
