Topics not referenced in TOC in a linked FM book

I have linked a FM book in RH where most of my topics are not referenced. It is a FM TOC, so all the headings 1 and 2 are included in the FM TOC. The topics are listed but are not referenced. The topic title has a "?" next to it. Also, all the actual heading in the topic appears as normal text instead of the Heading 1 or heading 2 paragraph tag. Can anybody provide any solution to this problem?

So you told RH to use your FM TOC right? Is there enough indenting within it to distinguish the Heading 1 and Heading 2 parts? That’s what RH depends on to figure out how to construct a RH TOC based on the FM one.

If your Heading 1 and Heading 2 paragraphs aren’t appearing in the RH topic correctly, check your Conversion Setting s to see how you told RH to map the FM para and character tags.
