Adobe's own Model Release being Rejected

I have built a library of images that I wish to upload and this time a first for me they contain an actual person. So to make things as simple as possible I got the model to complete Adobe's own model release. All the fields are completed and yet it keeps getting rejected with no explanation as to why - can someone help as i am just about to give up and load it elsewhere as support are no help either

Jumping out right now is All signatures must be dated and the dates must match.
