Is there an official model release form to use?

Can I use one of those free ones on the net and "fix" it following the requirements?

The answer is YES, and it is given to you to copy, fiil it out  and scan it whenever you are ready to send it. I wasted time searching for one asking and even made one and at the end at the moment I send it, I received the one from photoshop. And of course I had read the guidelines for Adobe Stock more than once.
