Do I need a license to sell self-portraits?

I was wondering if I need to fill the model license if I want to upload self portraits, and if I need a witness life if it was a normal model shoot.

You CERTAINLY need a model license if there is an identifiable person. The picture checkers have no other way to know that it is you. Witnesses is interesting. Probably, because that's how the paperwork is designed. This is a legal form, where the photographer and the witness can be held accountable if they are actually not telling the truth (which sadly happens a lot), so Adobe are unlikely to make any exceptions.
