Standard or extended license for public poster in medical field?

Let's assume a public advertisement (print) in form of a poster which is displayed in public.

The poster advertises a ophthalmologist/ophthalmologist office and its services.

The design includes an Adobe Stock of a person with glasses.

Is the standard license sufficient or an extended license required?

(Is 500.000 relevant here or whether the image of the person with glasses is the main value of the product?)

If you expect more than 500,000 views of the asset, an enhanced license is required.

Do the 500,000 views also apply to public displays? Which part of the license refers to a public display, is it a live performance?

"(i) you may not, in the aggregate, (A) cause or allow a Work to be reproduced on more than 500,000 printed
materials (including copies); and/or (B) incorporate a Work into a performance, broadcast, or digital production if
the audience is expected to be greater than 500,000 viewers. This restriction does not apply to Works that are
displayed on websites, social media sites, or mobile applications;"

Is your work getting integrated into a performance? Or a digital production? Or broadcasted?

I'm not sure about digital billboards, but IMHO they would be borderline between digital production and print.

(Disclaimer: As always with licensing, this is my interpretation of the rules. I think they are correct and advice is based on reading and interpreting the licence terms and on fair use for both the buyer and the artist/stock company, but I cannot rule out that my interpretation is wrong. I'm not an Adobe employee).

You do not sell the poster as a product and your print run is less than 500k, so a standard licence is OK.
