Hi.
I'm designing a single t-shirt for my personal use. No commercial gain or such. I found a picture that I want to use in my design. But I would like to change its colors to match my design and maybe alter it a bit. Can I use it for free if it's only for my own use? Or at least pay a one time fee? I don't want a monthly subscription for life to be able to finish my complete design just for one part of it.

You need to take a license for the image. However you can buy credits and pay the image with credits. One credit per standard license. Extended licenses will cost several credits.

As soon as you have taken the license it will be perpetual (not expiring) as long as you use the asset in the limits of the license terms. To be complete, it's not important if there is commercial gain or not, the usage determines the license.
