What are dimensions/resolution of document and files that you want to place? If you are placing the small image into huge document (much bigger dimensions) do not expect placed image to fit. It will fit when you are placing an image with bigger dimensions of document dimensions, not vice versa. The resolution also plays a role in this process: if you are placing image 2000x2000px with 300res into document 1000x1000px with 72 resolution it will not fit, it will be placed smaller because in the background Photoshop will match resolutions (with Resample turned on) and your image will be placed at something like 500x500px. ... Do you want to say that you have nested layer groups (groups inside group)? In that case right click on top level group (inside which lives all other groups) and choose: Delete Group. In pop-up which will appear choose: Group Only to get rid of top level group. Everything should work as you expect unless you still have nested groups.
