This issue is fixed in 23.5 which is available now. If you ever used transcription in 23.4 and got the relink dialog, you should close and not save the project. Once you save, the damage is done and you will have to restore from autosave backups. You should close the project without saving and update to 23.5. Now you can relink and everything will be correctly linked.

Premiere Pro will now replicate the folder structure exactly when importing folders. In previous versions of Premiere Pro, empty folders and folders containing only one file were not included as part of the import operation; the files would be imported, but not the folder itself. Now, in the latest beta build, 23.6.0 build 15 or later, all folders will now be included when importing assets using File>Import, Drag and Drop, and Media Browser.
In the example pictured. The following folders would not get turned into bins, now they will.
03 Graphics – Empty folder would previously get left out
04 Music – Folder with only 1 item would previously get left out
05 Photos – Folder with only 1 item would previously get left out
06 Documents – Empty folder would previously get left out
Now all these folders are turned into bins whether or not they contain 1 or fewer items.
Things to try:
Please import various types of media contained within folders.
Import deep folder structures
Methods supported:
File>import
drag and drop
Media Browser
In Import mode, this behavior is different. All folders are ignored and files are just imported in a flat list. This is the next step for us, to replicate the folder structure with Import mode. Stay tuned for more development on that.

@Daeshan5E55 I feel your pain, I really do, and I apologize for this unacceptable bug. I was an editor like you before coming to Adobe, and these kinds of bugs are truly infuriating. I assure you the team is fixing this as quickly as possible. The official build with this release will be out soon. Until then, I hope the workaround with beta will get you unblocked. I recommend saving a backup of the original project file you received. If you get the relink dialog in 23.4, don't save it. Follow the instructions on the linked post.

@rachelcenter We have a known issue in 23.4 that causes clips to get relinked incorrectly in precisely the way you describe, but it only happens when source transcription is present. It's possible you found another expression of this bug that does not require transcription, but I hope not. Can you please verify that no source clips have been transcribed? Do this by opening the text panel and setting it to the transcript tab. Then double click on source clips in the project panel to load them into the source monitor. If you see text in the transcript panel, you have transcripts generated. It's possible this got generated automatically in the background without you knowing. Maybe your collaborator did it? Can you try the beta and let me know if it helps? FYI, if you see the problem in 23.4 and save the project, the damage is done and cannot be fixed by going to the beta. Make sure you are opening a known good project file. So – if you have a properly linked project functioning as expected on your system, you can send that to your collaborator and have them open it in Beta (or vice versa). I hope this helps.

This issue has been fixed in public beta as of 23.6.0 build 02 released Thursday, May 25th. I posted the details in this known issue doc but will put the instructions here as well. If you get the media relinking dialog in 23.4 and you have previously used transcription, close and do not save the project. Make a backup of your project file just to be safe. Use public beta to fix your project.
Download Premiere Pro beta version 23.6.0 build 02 or later
Open in beta, reconnect the media, and ensure that the sequence looks correct.
You may choose to just work in Beta –or– save, and open in 23.4

This issue has been fixed in 23.5 which is now available. Please update to get the fix.
We are aware of an issue in Premiere Pro version 23.4: when transcribed clips in a timeline are taken offline and relinked, clips shift and reset to the first frame. Effects, transitions, and speed changes can also be removed or impacted. Our engineers understand the problem well, and we are testing fixes internally right now. We have put a fix into Public Beta and will get the fix into an official release ASAP. I will update this post with more details as soon as I learn them.
If you are experiencing this problem:
If you get the media relinking dialog in 23.4 and you have previously used transcription, close and do not save the project. Make a backup of your project file just to be safe.
Workaround #1 – Restore media
Put the media back in its original location so no relinking is required.
* There are many cases where this is not possible; try one of the following:
Workaround #2 – Beta
Download Premiere Pro beta version 23.6.0 build 02 or later
Open in beta, reconnect the media, and ensure the sequence looks correct.
You may choose to just work in Beta –or– save and open in 23.4
Workaround #3 – XML
Export XML (File > Export > Final Cut Pro XML) from the original system that has properly connected media.
Import the XML into a new project on the new system and reconnect the media
* Please take note – not all aspects of the sequence will be reproduced by the XML
Workaround #4 – revert to Premiere Pro 23.3
Open Creative Cloud Desktop app
Click on All Apps
Find Premiere Pro
From the three dots on the right, choose “Other versions”
Choose 23.3

Here is today's update on this issue. Our engineers have found two bugs responsible for this behavior. Clips containing source transcription are being read with the wrong duration during the relink process which causes the source clips and edits to get reconnected incorrectly. They have a fix that we are testing internally and will roll out to beta as quickly as possible in the next couple of days. Once we do that, the beta app will be the secret weapon to fix this issue. You can open the project in beta, relink, save, and open in 23.4 and all will be well. Obviously, we plan to release an official release version with this fix ASAP. I'll update this thread when the beta version containing the fix is available.

@BetaTesting editor - unfortunately, the state of the "preserve transcripts" option in the relink dialog doesn't change the behavior here. It's not a solution.

@Stan Jones thank you very much for the links. I've been looking at many of those already and I'm trying to figure out if they are all the same problem or different issues. Internally we have definitely discovered an issue in 23.4 related very specifically to reconnecting clips containing a source transcript.

"Unknown" either means you had the setting to identify speakers disabled or the AI model can't clearly hear the speakers and tell them apart. You should see "Speaker 1", "Speaker 2" etc. Try re-transcribing your source clips.
Be sure you are in the Tect-based editing workspace - Window > Workspaces > Text-based editing
Find the source clip in the project panel and load it into the source monitor by double clicking
In the text panel, make sure you are on the "Transcript" tab. Choose "Re transcribe" from the overflow menu (three dots)
Make sure to check the option to separate speakers Is the audio recording clear? Are the different speakers clearly audible and have different-sounding voices? Sometimes voices that sound similar get lumped together. If all else fails, you can identify the speakers manually.

You need to update to a newer version of Premiere Pro. Which version are you running? All transcription is now running on your local computer and does not need an internet connection. This is a preference that we changed a few releases back. Most users were confused about pasting and track targeting and expected clips to paste to the same track they came from. But you can change it back to how it was in the keyboard shortcuts to follow track targeting. Keyboard shortcuts
