I am not sure but unless the item is not phrased as a question, or does not have question in the Content it doesn not become active. I have seen this several times.  And its always appeared for me when the person was just declaring a statement. and not asking anything.

Probably after it has been fixed, but as of 3:08PM (PDT) May 05, mine is working. Though I have been on the forum most of the morning, I did not notice this being missing, but have not needed to attach an image. Hope it was just Adobe maintenance and not a foreboding omen.

It was basically a typo.
