Hi, how does one use an extension once it is loaded? I see the "Manage Extensions" command but no clue is there on how to access it..

manager. Click on a listed installed extension , on the bottom of the
extension manager it will usually mention where or how to access that
extension in dreamweaver.

Normally you should be able to access the extension from the Commands menu or the Objects/Common tab.

I have CS3. I don't have any access on the Commands menu and I don't know what the Objects/Common tab is.
