Exporting fillable fields in InDesign

I created fillable text fields and checkboxes in my InDesign document and I'm trying to get them to appear as fillable fields when I export to a pdf file. Currently the checkboxes only appear as frames, but without being able to check or uncheck them and the text fields don't appear at all. I selected "Include Appearance" in the Interactive Elements field in the Export dialogue. I'm including screenshots of how I'm formatting the fields and the options I've selected for the export. Thanks for your help!

Great, that worked thanks Bob! For some reason I was looking in the parameters once I had already chosen to export it as a printable pdf, but had skipped the step of changing the file type in the dialogue box just before.
