Insert a logo into a fill-in form - Add'l Information

I need to set up a form so clients can insert their own logos in the following formats: eps, jpeg, tif. I'm using Acrobat X Pro.  How would I go about that? Thanks!

Note: The clients will be using the free Adobe Reader to insert their logos.
